What conditions can chest tightness indicate?

Chest tightness and difficulty breathing can result from several disorders. The most common include:

 

Coronary artery disease

If you have chest tightness and shortness of breath, it may signal a risk of coronary artery disease caused by fatty and calcified plaque buildup that reduces blood flow to the heart muscle. Early on, symptoms may be gradual—pressure on the left side of the chest as if something heavy is pressing down. As it worsens, pain can radiate to the jaw or left arm, with shortness of breath and sweating, increasing the risk of fainting or cardiac arrest.

 

Acute myocardial ischemia

Myocardial ischemia often occurs suddenly, causing chest tightness, breathing difficulty, sweating, and shortness of breath. It is triggered by a blood clot blocking an artery already narrowed by plaque. This is dangerous, can occur without warning, and may lead to heart failure.

 

Pulmonary embolism

Pulmonary embolism occurs when a blood clot that formed in a vein of the leg or arm—connected to the right side of the heart and the lungs—travels to and blocks an artery in the lungs. Symptoms include easy fatigue, chest tightness, trouble breathing, dry cough, lightheadedness, dizziness, palpitations or rapid heartbeat, and it can be life-threatening.

 

Gastroesophageal reflux disease (GERD)

A very common cause is acid reflux, from stomach acid, gas, or food flowing back into the esophagus. This leads to burning pain in the chest or throat and sour burps. Treatment is needed to prevent chronic symptoms.

 

Costochondritis / chest wall muscle inflammation

Sharp chest pains on the left, right, or center near the ribs—sometimes with a burning sensation at the chest or pit of the stomach—can worsen with coughing, sneezing, or movement. These often stem from inflammation of the chest wall muscles due to sports or heavy use. It is treatable but may recur.

 

Asthma

Chest tightness and breathing difficulty can also be due to asthma, typically with a chronic cough, wheezing, or shortness of breath. Proper treatment and medication are essential because asthma can be fatal.

 

Costal cartilage/rib inflammation

Injury to the chest or strenuous activity—heavy lifting, over-exercising—as well as arthritis, joint infections, or tumors at the rib joints can cause rib inflammation and stabbing chest pain. Proper diagnosis is required to identify the cause and guide treatment.

What to do when you have chest pain and difficulty breathing

 

  • Stop strenuous activity immediately—walking, running, cycling—and sit down to observe symptoms.
  • Breathe in slowly and deeply, hold briefly, then exhale gently through your mouth while lightly massaging to relax the muscles.
  • Reduce stress, which can trigger chest tightness; practice slow deep breathing to help normalize heart function.
  • If symptoms are severe, call for help or contact a hospital right away.
