What to do when you visit the Heart Failure Center
- Register, measure blood pressure and vital signs, draw blood, weigh, and measure height
- Prepare comfortable walking shoes, wear loose-fitting clothing, such as short sleeves and stretchable pants, for the 6-Minute Walk Test
- The nurse at the Heart Failure Center will inquire about symptoms including: emergency medical history, hospital visits before appointments, chest pain, fainting, palpitations, shortness of breath, difficulty sitting or standing at night, fatigue, inability to lie flat, swelling in the legs, stomach bloating, nausea or vomiting
- Provide guidance on lifestyle changes, diet, and basic exercise, review flu and pneumococcal vaccination history, smoking cessation, and alcohol moderation
- Provide understanding of heart failure, diagnostic tests, and treatment approaches through educational videos and manuals on heart failure
- Pharmacist at the Heart Failure Center
- Verify correct medication usage
- Check for side effects or adverse reactions
- Inquire about other medications that may interact with current prescriptions
- Offer advice and instructions for medications at home
- Always bring your medications when visiting doctors and pharmacists
- Specialist cardiologist will assess symptoms and signs, recommend additional tests as needed, such as Echocardiogram, CT-Scan, or CMR
- Referral to the heart rehabilitation team and assessment for CPET to recommend exercise or provide prognosis for some patients
- Provide treatment
- Gradually adjust primary medications every 2-4 weeks to reach the maximum appropriate dosage as per treatment guidelines or as suitable for the individual case
- Monitor kidney function and electrolyte levels during medication adjustments
- Monitor and manage comorbid conditions such as diabetes, hypertension, and hyperlipidemia
- Meet with the pharmacist and nurse after seeing the doctor, for follow-up and advice
- Reinforce recommendations before discharge
- Provide guidance on changes in medications
- Offer instructions if starting new medications
- Provide additional treatment as prescribed, such as vaccinations or nutrition assessments
- Refer to heart rehabilitation team or schedule future appointments
- Return home, adjust diet, exercise, and complete your medication regimen
